Release and Medical Authorization: The release and the medial authorization must be signed by a parent or guardian in order for student to participate in camp activities. Release and liability In consideration for the MiJo Sports & Entertainment Youth Camp granting the student permission to participate in the football camp, I hereby assume all risks of his personal injury that may result from Sport related activity. As parent/guardian I do herby release MiJo Sports & Entertainment and the MiJo Sports & Entertainment Youth Camp and their officers, employees and agents and all instructions and all participants in said football camp program from all liability, including claims and suits at law or inequity, for injury which may result from the student taking part in the sports camp. Medical Authorization I hereby authorize and give my consent to the health authorities of MiJo Sports & Entertainment or any licensed physician or athletic trainer to perform upon or administer any reasonable, necessary medical treatment to: I agree to assume all costs related to such treatment. Photo/Video Authorization: I hereby grant MiJo Sports & Entertainment (the “Agency”) the irrevocable right and permission to use photographs and/or video recordings of my child on Agency website and in publications, promotional flyers, social media or for any other similar purpose without compensation to me. I understand and agree that such photographs and/or video recordings of my child may be placed on the Internet. I also understand and agree that I may be identified by name and/or title in printed, Internet or broadcast information that might accompany the photographs and/or video recordings of my child. I waive the right to approve the final product.
